The Department of Family Medicine is a dynamic department with a progressive vision. It is
committed to providing excellent, comprehensive, and continuous primary care, outstanding
medical education, and conducting community-based health research.

In its continuing efforts to impact common health problems (hypertension, diabetes, depression,
and immunizations), the Department of Family Medicine uses a chronic disease management
model. This model empowers patients to manage and control their medical illnesses.
Outreach and collaboration with community based organizations are central to the Department
of Family Medicine’s mission. The collaboration is in the form of delivering health services,
research, and the development and expansion of existing programs. The department’s creative
geriatric home care program for the vulnerable elderly is a model for today and the future. |
| Family
Medicine has also put into place a cutting edge,
Community Health Worker program, which epitomizes
the essence of community and home based health
care.Historically, Family Medicine has worked closely with the
Department of Dentistry, referring patients for dental care as
well as working collaboratively at the Ogden Family and Dental
Health Center. |
