BronxCare Health System Receives Gold Plus Quality Achievement Award for Heart Failure Care

Mission Lifeline Quality Achievement Awards

The American Heart Association selected BronxCare Health System for its Get With The Guidelines-Heart Failure Gold Plus Quality Achievement Award and Mission Lifeline Quality Achievement Award. “We are pleased to recognize BronxCare Health System for their commitment to heart failure care,” stated Paul Heidenreich, M.D., M.S., National Chairman, Get With The Guidelines Steering Committee and Professor of Medicine at Stanford University.

BronxCare Health System earned its award by meeting specific quality achievement measures for the diagnosis and treatment of heart failure patients. These measures include evaluation of the patient, proper use of medications, and aggressive risk-reduction therapies. Patients also receive education on managing their heart failure and overall health, and get follow-up visits scheduled, as well as other care transition interventions.
