In this role, you are responsible for identifying, implementing, monitoring and evaluating clinical and operational quality improvement activities and providing related consulting and support services for Health Home Program and Care Management Partners. Contributes and participates in the data collection, analysis, implementation of and compliance with risk management and claims activities. Supports and participates with the Continuous Quality Improvement (CQI) teams.
Requirements: Five (5) years of experience in healthcare setting and progressive administrative leadership experience in quality improvement. NYS Registered Nurse, Master's required, MS Healthcare Quality preferred.
Requirements: NYS Registered Nurse license, BSN, BLS, and a minimum of one year (recent) Interventional Radiology experience

Will utilize Care Management to conduct assessments for admissions to the assigned unit and determine Care Transition and Discharge Planning needs. The CM, leads and participates in inter-disciplinary rounds, as well as participate in departmental meetings, training programs, supervisory sessions, and staff meetings. Work with family and significant others to provide education related to disease state and chronic conditions. Utilizes Sunrise Care Manager platform to monitor DRG, MS-DRG and GMLOS as a guide to determining appropriate condition related Length of Stay.
Requirements: NYS Registered Nurse license, BSN, BLS, minimum of one year (recent) experience as a Care manager in an acute care setting. CCM preferred or within 3 years.
The Nurse Practitioner will provide clinical care primarily to ambulatory surgical patients and will coordinate the designated programs as an active member of the Department of Surgery. Some cross coverage may also be required.
Requirements: NYS Acute or Family NP license and certification; Current BLS; Spanish speaking required
