Director of Ambulatory Nutrition

BronxCare Health System, a major teaching hospital system, now has an excellent opportunity for a Director of Ambulatory Nutrition in its Department of Population Health, to lead a team of diabetes educators, in collaboration with primary care physicians and endocrinologists. The Director of Ambulatory Nutrition will provide patient care and administrative leadership at BronxCare’s ADCES-accredited Diabetes Center of Excellence. Additional opportunities in this key position include coordination and participation in community outreach initiatives, as well as support for grant-funded projects & research/ mentorship of medical students, residents, and dietary/nutrition interns.

Ambulatory Care Pediatrician

Infectious Disease Physician

BronxCare Health System, a major teaching hospital and health system, now has an excellent opportunity for an Infectious Disease Physician. Responsibilities include conducting inpatient and outpatient consultations, as well as HIV primary care. Infectious Disease Physicians participate in the teaching and administrative activities of the Division of Infectious Diseases and Department of Medicine. Academic appointments are provided at the Icahn School of Medicine at Mount Sinai. Leadership opportunities include overseeing service programs, such as case management, counseling and testing, linkage to care, and community outreach. Infectious Diseases Physicians are also participants in antimicrobial stewardship.

BronxCare serves a population with one of the highest rates of HIV prevalence in the country. Our Center for Comprehensive Care provides co-located case management, care coordination, women’s health, behavioral health, and specialty care for adult and pediatric patients. The practice also manages a large and growing Travel Clinic.

Social Workers

BronxCare Health System now has important opportunities at the outpatient and inpatient levels in its Department of Social Work. At BronxCare, Social Workers lead, learn, and educate, as well as collaborate across the full spectrum of key areas, providing discharge planning, transition of care, and education.

  • Outpatient Social Workers (two positions available)
    Under the direction of clinical supervisors, Outpatient Social Workers are responsible for determining and assisting in the resolution of patient/family issues and working with an interdisciplinary team in providing patient care, preparing statistical reports, and serving as a liaison within the department, hospital, and with community agencies. Activities and related efforts will also be directed to assuring the smooth functioning of the social work program.

    Master’s degree from an accredited Graduate School of Social Work and a minimum of two years’ post-graduate experience in providing social work services within a hospital, medical, or mental health setting are essential. LCSW/LMSW required. Bilingual abilities are also preferred.
  • Inpatient Level (three positions available)
    Inpatient Social Workers will utilize their experience in providing psychosocial services as well as coordinate discharge planning and a continuum of care for patients and families. Inpatient Social Worker will also assist in enhancing the capabilities for assigned patients in coping with illness, crisis situations, and daily functioning.

    Master’s Degree in Social Work (MSW) and New York State License as an LMSW are required. Limited permit is needed for newly graduated social workers (must provide exam date prior to start of employment). At least one year of prior Social Services employment preferred.
