Primary Care and Preventive Medicine

The Division of Primary Care and Preventive Medicine, under the leadership of Dr. Abayomi Salako, is taking a pivotal role in delivering comprehensive patient-centered primary care. “We are a conduit for a health care delivery system that aims to improve health outcomes for our patients and community,” stated Dr. Salako. As part of its commitment to strengthen primary care, the department has focused its efforts on chronic disease management, wellness, and promoting cancer screening. BronxCare is a key stakeholder in a remote blood pressure monitoring and screening program for Hepatitis B, with linkages to care as part of New York City’s efforts to eradicate Hepatitis. It is also a strong advocate for the age-friendly initiative that provides care for older adults. In addition, the division is addressing health literacy issues through a Community Health Worker initiative. Ambulatory Care education is also being provided to medical residents on a continuing basis, thereby helping to train the primary care workforce of the future.
